Three Rivers Midlands accepts referrals from parents and family members, professionals, schools, physicians and providers from the community, both in state and out of state.

Admissions are conducted Monday through Friday.
When making a referral the following information facilitates the admission process:
- Psychiatric evaluation (within six months)
- Presenting problems and history of treatment
- Clinical records (last six months)
- Medical and immunization records
- Psychological history (last six months)
- Medical, medication and dental history
- School records (IEP, 504)
- Insurance information
Admissions staff will review with the team to determine if a youth will benefit from our program.
