Criteria for Admissions

There is a level of criteria that must be met for admissions at Three Rivers Midlands Residential Treatment Center. When considering residential services, please keep in mind the following:

• Three Rivers Midlands Residential Treatment Center is the highest level of care for emotional and behavioral psychiatric conditions
• Outpatient services have ceased to benefit the patient
• The patient is experiencing ongoing emotional stressors
• The patient’s primary diagnosis is emotional/behavioral

Contact our Admissions Department to determine if the patient meets criteria.

Medical Records and Release of Information

To request a copy of medical records or authorize Three Rivers Midlands to share information with another provider, agency, or individual, please complete the Release of Information form.

All required sections must be completed and signed before the request is processed. Submit the completed form using the contact information provided on the form. Medical Record Fax Number (803) 727-1364
